Placing an Order: If you place an order from Omega, we (or our authorized third-party payment processor) will collect information such as your first and last name, email address, phone number, mailing address, delivery address, and your payment information (including billing address, credit card number, expiry date and CVV/CVC code) in order to process the transaction.

If your physician or pharmacist places an order for an allergy or other product on your behalf, we may in these limited circumstances collect additional information about you, including health- related personal information: specifically, the prescription as written by your physician, which includes your name, date of birth, and information about the prescribed product. We also print this information on a label and affix it to the allergy product before mailing the product to your physician or pharmacist, so the physician or pharmacist can identify the patient associated with the order. We collect and process this information solely for the purposes of fulfilling orders for allergy products.

Contacting Us: When you contact us with a comment, question or complaint through email, telephone, the contact us form on our Website, you may be asked for information that identifies you, such as your name, email address, along with additional information we need to help us promptly answer your question or respond to your comment.

If you contact us to report quality issues or side effects (such as adverse drug reactions or adverse events) related to your use of our products, you may be asked for additional information to allow- 2 - us to better understand the nature and circumstances of the issue.
